In Pakistan, i have not seen any such app in the restaurants for feedback collection. Feedback is an app for restaurants owners to collect customer feedback via smart phones. The app has a simple entry form and summary area where user can view the feedback results in graphical format.

The app is for restaurants and dinning placing. Feedback helps the restaurants manager to monitor the performance instantly, by eliminating the two level data entry process. Once the customer enter the feedback it automatically start showing in the report area of the app. 

This version of the app offers very limited output but we are working on its 2nd release expected to be publish next month, in which the report section have more charts and analysis plus user can view it over internet.
